The List
Here's how it works: We send a survey to each doctor on the New Hampshire Board of Medicine's list of physicians licensed to practice in the state. This year, we received responses from 429. The survey asks the physicians to nominate specialists they would most recommend to family and friends. Hundreds of physicians statewide were named. We include those who received the greatest number of recommendations within 45 specialties. Those picked in our survey are then fact-checked for biographical information. New Hampshire Magazine editors are not able to visit the Top Doctors nor screen their credentials. Many excellent physicians are not on this list and we wish we could report on all the stories of tireless service to patients that we hear during the course of our poll. Many fine doctors have practices in areas of the state with small populations and fewer associates to receive surveys, thus creating a bias toward more populated regions of the state. Even so, smaller hospitals are often represented in the poll and we trust this list is broad enough to be useful and to represent the level of excellence that exists across the state. When seeking a new specialist for care, it's always a good idea to consult with your own doctor first.
In our 2010 survey we asked New Hampshire doctors who they would choose to provide for the medical needs of friends and loved ones and their choices are here in your hands. To broaden the reach of our poll, we list our top doctors along with other leading physicians in each specialty. Plus, we solicited votes from our readers.

Several of the top vote-getters are profiled to help explain the challenges and rewards of medicine, and we include replies from doctors to two questions we asked on the poll about health care reform.

Why medicine? Why this specialty? I did not know from an early age that I wanted to go into medicine. However, as one of eight kids, I realized early on that "service" was my middle name. I still feel that medicine is ultimately a service profession, and that is what motivates me to continue.
Any misconceptions about your specialty? All neurological diseases are fatal or incurable, right? Wrong! Many patients have symptoms which, after an Internet search, they conclude must be due to some terrible neurological illness, when in fact there are other very simple and reassuring explanations. In neurology, as in other parts of medicine, many diseases have very effective treatments, or are controllable with medication or procedures. Armed with an understanding of their neurological disease, together with effective symptom management, most patients continue to find life very much worth living.
What do people need to understand about medicine? Despite our continuing efforts, medicine is not an exact science. We are getting much better at evidence-based treatments, but people need to understand that even if the majority of patients do well, that still does not mean that an individual will have the same level of success with that treatment. Also, people need to recognize that medicine can delay death sometimes, but we all die eventually. In this day of medical miracles it is more important than ever to plan for death, as scary as that sounds. Advance directives, with living wills and the appointment of a durable power of attorney for health care, can greatly ease the anguish many patients and families feel at the end of life.
Most rewarding aspect? The opportunity to cut to the chase and deal with matters patients are genuinely concerned about, and which really matter to them Do you have a motto or philosophy? Life is short, so live it!

Most rewarding aspect of your work? I feel very fortunate to be in a position where I can help people who are in pain or have been injured and cannot function at work or play.
What makes a great doctor? I believe a great doctor is both highly competent and inherently driven to learn and progress within his/her field. In particular, orthopaedic surgery and sports medicine knowledge is expanding at such speed that what we believe we know one year may be irrelevant or even wrong the next. The "great" doctor will be the doctor who has an insatiable appetite for learning. I also believe that there is no technical skill that is more important than listening. Finally, I believe that there are no attributes more important than empathy and compassion.
Any misconceptions about your specialty? Misconceptions about orthopaedics are that we only treat broken bones or surgical problems such as torn ligaments and tendons. However, orthopaedic surgeons are trained to treat primary musculoskeletal problems such as tendonitis, simple sprains and strains, osteoporosis and even concussions within the sports fields.
Exciting development in your field? The introduction of biologics into orthopaedics. One example is the use of platelet-rich plasma to exploit the benefits of growth factors and the healing potential of the patient's own blood to stimulate healing of tendons and ligaments. Other examples are the biologic augmentation of rotator cuff repairs and growing patient's own cartilage cells to replace focal areas of arthritis. The goal of these advancements is to someday make joint replacements of historical significance only.

Why medicine? Why this specialty? I chose medicine because I thought it would best utilize my skills and it was my desire to find a specialty that allowed me to "fix" things in a tangible way. Orthopaedic surgery seemed like one of the few places in medicine where excellent outcomes could be expected and where the results were almost immediate and very tangible.
Most rewarding aspect of your work? The best part of my job and the thing that will keep me doing it is the satisfaction I get from significantly improving somebody's quality of life and allowing them to pursue their passion(s).
Most difficult aspect? Making time for myself and my family; saying "no" to more work.
What makes a great doctor? The ability to empathize and connect with patients of all different backgrounds to earn their trust; determine what is most important to the patient and be true to that; be systematic and consistent; and expect perfection but be realistic and practical.
Rx for staying healthy? I recommend that all patients develop a passion in something or some activity that involves physical activity; this is good for both physical and mental health. No matter what the season, there is something for everyone.
How do you put patients at ease? Humor (often at my expense); figuring out what someone is interested in and engaging them about it.
Exciting development in your field? Minimally invasive direct anterior total hip replacement.
What can patients bring to the patient/doctor equation? Do your research ahead of time and write down questions on note cards or whatever works. Never try to make a big health decision without your spouse or most important significant other.

Why medicine? Why this specialty? I decided to become an animal doctor when I was 12, as I was helpless to save a puppy that was killed by a car as I stood watching; My mother suggested "doctoring humans" as veterinary medicine in India is mostly large animal husbandry; I was always fascinated by blood as an organ in medical school and chose hematology, which is linked to oncology.
Most rewarding aspect of your work? The most rewarding part of oncology is to help people not suffer from cancer, but help them enable themselves to live as fully as possible for the time that they have.
Exciting development in your field? Oncology is making leaps and bounds in research; Eventually, I hope to see it as a field where cancer is treated like a chronic disease (like HIV), have the ability to see who may benefit from one kind of treatment versus another, and use a series of protein blockers to inhibit growth of cancers.
What can patients bring to the patient/doctor equation? I think of the patient-MD relationship as a team effort with mutual respect and trust; If the balance is lost, it is important for the patient to find a provider he/she can trust. Patients should be able to ask questions and feel comfortable in the plan of care, and patients must also understand that all physicians do not have all the answers.
What do people need to understand about medicine? I think it is very important for patients also to be aware of the cost of health care and not ask for redundant tests. Similarly if an extra 15 minutes of time will enable a physician to explain one's thought process, we may be able to save on unnecessary tests. We all share the blame for costs. Do you have a motto or philosophy? My basic philosophy is to treat patients as I would like to be treated.

Why medicine? Why this specialty? The ability to combine my love for math and science with an end result of actually helping people convinced me at an early age that I wanted to become a doctor. It was not until I was in medical school and was exposed to Pain Medicine in a very early stage of the subspecialties development that I concluded that relieving pain and suffering sounded like a terrific career choice and it has been.
The most rewarding aspect of my work is to see the fear and anguish of my patients suffering resolving and to see them transported back into their old selves, once again smiling, happy, and doing the things that they used to be able to do. The most difficult aspect is realizing that there are still many patients with chronic pain whose pain we cannot relieve and trying to help these patients get on with their lives is emotionally draining not only for them and their families but also for their doctors and nurses.
What do people need to understand about medicine? I think that one of the most important, and perhaps the most difficult to accept fact is, that medicine is imperfect. We are making advances and we are able to do much more now than we were able to do 20 years ago but people are still dying of cancer and people are still suffering. The tools we have are inadequate to enable us to help everyone. We should all support medical research to whatever degree that we can. What do you do to relieve stress and leave work behind? It may be becoming something of a cliche, but I am a rock and blues guitarist and play in a band one night a week. I am in a men's book club. More than reading and music, I have been blessed with a loving wife who helps to calm and distract me.

Why medicine? Why this specialty? My grandfather, Dr. Rolf C. Norris, was a quintessential physician and served as a great role model during my youth. After Dartmouth Medical School, I was initially interested in family practice. But during a rotating internship, it became clear to me that caring for children was what I really wanted to focus on. Kids are inspiring!
Most rewarding aspect of your work? I often feel good at the end of a busy day knowing that I may have made a positive difference in someone's life circumstances.
Most difficult aspect? The most difficult aspect of my job is dealing with the death of a child. No one expects children to die. But we have lost patients to cancer, accidents and suicides. Such events are rare but devastating to the entire community.
Rx for staying healthy? Keep it simple Limit screen time Get outside Make time to enjoy meals with family How do you put patients at ease?
